Resumen

Objetivo Compartir la experiencia del manejo clínico para reducir la mortalidad de pacientes ancianos con fractura de cadera. Métodos Se recopiló los datos de pacientes ancianos con fractura de cadera ingresados en nuestro hospital en el período enero-diciembre de 2018, incluyen el sexo, la edad, el tipo de fractura, el número de comorbilidades, el momento quirúrgico, el método quirúrgico, la clasificación de estado físico de la Sociedad Americana de Anestesiología (ASA) y estado de supervivencia. En combinación con las guías actuales para el manejo de la fractura de cadera, intercambiamos la experiencia del manejo perioperatorio y rehabilitación postoperatoria. Resultados Se seleccionaron un total de 172 pacientes, incluidos 53 hombres y 119 mujeres, con una edad promedio de (78.4±6.8) años; 97 fracturas del cuello femoral y 75 fracturas intertrocantéreas femorales; 78 pacientes con menos de 3 comorbilidades, 94 pacientes con 3 o más comorbilidades; 64 casos de ASA grado III y IV, 108 casos de grado I y II; 83 casos de cirugía temprana y 89 casos de cirugía retrasada; con una estancia hospitalaria promedio de (14.1±3.1) días; 9 casos murieron dentro de 1 año, y la tasa de mortalidad fue del 5,2%. Conclusión Aunque los pacientes con fractura de cadera son mayores, tienen más comorbilidades y mayores riesgos quirúrgicos, podemos reducir en gran medida la mortalidad mediante un manejo perioperatorio científico y cuidadoso, rehabilitación postoperatoria y educación sanitaria.
